Hi there,
Have you selected that question group within the event editor?
Each event can have different question groups selected so you’ll need to enable that group for at least ‘Primary registrants’ for it to show.

Hi Steve,
You need to assign the question group to the events.
For that, you need to edit the event and in the bottom right corner you can find the option to assign the question group to the event:

Thank you. I only do this once a year and I completely forgot how to do that.
One more question. I decided the description would be the best place to add the “terms and conditions” block. But is there a way to make this type larger and in black? Is there a way to edit the css for that text block?

To target that specific text block you can use:
.ee-reg-qstn-16-input-dv .ee-question-option-desc {
font-size: 1em;
color: black;
}
Add that to Appearance -> Customize -> Additional CSS.
