Can't redeem my $70 Support ticket

I attempted to redeem my support ticket. I do not understand the required fields as I am a small business owner, not an IT person. I clicked on the link to explain the terms and they were not defined.

Hi,

For the “WordPress Dashboard (WP-admin) URL*” field, your website domain will be sufficient. The temporary username and password fields will need to be provided so we can access the WordPress Dashboard.

You can contact your web host to get the required file access information, or FTP.
