The data entry fields for the default personal information and default address question groupd do not display, although the titles of the default question groups do display.
A custom created question group does display data entry field.

Could you login to your WP dashboard and go to Event Espresso –> Registration Form?
Are the questions appearing there?
Also, is this a windows-based server?

The system questions do appear in Event Espresso -> Registration form.
The site is hosted by PAIR networks; I use Windows 8.

Hi Valerie,
Can you go into the Registration form question editor and re-save the questions and their respective question groups? It appears this site was previously running Event Espresso 3 and the questions may not have migrated correctly. If re-saving the questions does not make them appear, you can send WordPress admin level log in credentials via the contact form on this page and we can investigate further:

When resaving the questions, I noticed that I could select questions to add to the system questions; and “first name” was not show on the Personal Information question set. I added that question manually, and now the personal and address fields display. Since this was a hack, I’ll test it to make sure the form works, but for now, I’m OK. Thanks for your help.

Hi Valerie, I had a question about your prior reply.
When you were viewing the personal question group within your WP dashboard, were the personal questions such as first name, last name, and email unchecked?

In the personal question group, only “last name” displayed and it was not checked.
Email and First name were not listed. I added “first name” as a custom question then added it to the Personal Question Group, but now I find it’s (first name) not displaying on the receipt and registration pages.

Thanks Valerie. This isn’t fully resolved as a custom question would not be shown in the expected places (i.e. Event Espresso is set to show information such as contact name from system questions).
The personal questions along with the address questions are considered system questions and it sounds like one or more of those did not setup correctly.

We’ll need access to the database preferably through PHPMyAdmin or an equivalent tool. Then we’ll try adding any missing system questions.
Could you also let us know when you migrated from Event Espresso 3 (if you know that information)?
