RADIO_BTN not accepting selection

We have a required question with radio button options, and no matter what selection is made, our customers are being stopped by the following message:

EED_Single_Page_Checkout – _check_form_submission – 934

Hi Jason, are there any errors appearing on the page when you inspect the page using Chrome developer tools, Firebug, or another tool?

I just tested the recent release of Event Espresso 4 and set all questions (one for each question type) to required and did not see this issue:
