The form captures the name, email address, credentials and phone number of the registrant. I need it to capture some other elements (like their address and med license number) but I can’t seem see how to add that secondary form to this page.
I can see two question groups: “Personal Information” and “Address Information”. I can’t get the Address Information to appear on the registration page.
Each event allows you to select the question groups requested for both the primary registrant and any additional registrations.
Edit the event and look in the sidebar, you’ll see a section that allows you to select the question groups, like this – http://take.ms/1CBvc
Have you enabled the ‘Address information’ question group on at least the primary registrant?
