I noticed that if you add a registrant manually from the backend that it doesn’t create a user for that new registrant. How do I make sure a user is created? From the front end, when someone registers for an event it does create a user effectively.

Hi there,
The EE4 user integration add on does not create users from the registration details entered within an admin registration, one reason being that the contact would be assigned to the admin account rather than the new user.
If you want to create users from registrations you will need to add the registration from the front end whilst not logged in, you could use a chrome incognito window and register onto the event using that to create the user.
