When registrants go to the event and click “register” to register, the fields for the personal information are missing, and I found out that if the fields from inside the Manage Questions page are NOT required, the fields show up. However, if they are marked as required fields, the text fields do not show up. The page I am referring to is http://evillestudios.com/tickets/

Your theme is hiding the required fields in the css file (screenshot: http://www.screencast.com/t/XacfP0ygS)
Also, looks like you are getting a major Javascript error (this is from the home page):

What ever that LoginRadiusShare script is, it is messing up your entire page. It is declaring the <!DOCTYPE html in the middle of the page:
