Caching can cause all kinds of weird and wonderful issues.
Depending on the type of caching in use it can break the user’s session, more than a single user has the same session and you effectively overwrite another registration because the sessions match.
Note I’m not saying that is what actually happened, simply that it is possible, we don’t have enough information to know what has happened.
You’ll want to exclude EE’s critical pages from caching:
Which email did the event author receive? Registration Pending Payment? Registration Approved? Another?
If you go to Event Espresso -> Messages -> Message Queue, can you see the email sent to the event admin (and registrant) for this registration?
