The only time this can happen is if the user gets to the payment screen of the registration process and does not pay or finalise the registration and then proceeds to go back to the start and register someone else.
This will then over write the original registration (even though emails have gone out), as it was not completed.
We added in a JavaScript warning to help reduce this issue, but if the user either ignores it or does not have JavaScript on it could still occur.
The only other thing it could be is if an admin deletes and attendee or if the attendee has access to cancel their booking.
