Hi,

Creating a question does not automatically enable that new question for events.

That question will need to be added to a question group and then one of your events should have that question group enabled through the event editor:

Can you share a link to the event where that question is displayed? If not, can you explain how it is visually different?

There seems to be some conflicts with your theme’s CSS and EE’s styles. You could try adding this to your theme’s style.css file, or a custom CSS plugin:

.event_espresso_form_wrapper .radio-btn-lbl input {
    top: -6px;
    width: 10% !important;
}
