Personal Information fields are missing

When you purchase a ticket at http://www.racenrock.com/tickets the personal information fields are missing. The user can’t input any of their information. Help! My event is very soon and this issue just appeared.

How did you fix your problem? I am having the same issue.

I had done a display: none; in the css that got rid of the dates on each of my event but also made the personal information fields disappear as well.

Ah, wish it was that simple for me… I cannot figure out why no personal questions show up at all and it’s not just a display issue. Thanks for the info on what you did though.
