All the details of my problem match the original poster’s problem. Upon choosing the featured image and clicking “Insert Into Post”, the AJAX GET request is sent to get the image and the image is returned properly. However, after the image is returned another GET request is sent to
[site_url]/wp-admin/undefined
And, obviously there’s a 404 response. Given the “undefined”, I’m assuming this is related to some improper variable definitions in the JS, but I’ve been unable to figure out where this bad AJAX request originates from. After this, the page attempts to add the image to the featured image box, but creates the following element:
And upon saving the event, any trace that an image tried to be added to the event is lost, except for the image in the media library. I’ve set the permissions of the wp-content directory and all subdirectories to 777, so I do not believe this to be a permissions error.
After reading further posts in the original thread, I created a new user account and can attach featured images using the new admin account, but am still unable using my old account. I’ve sent in the login details for both accounts so that they can be tested and examined independently.
The issue does not seem to be browser dependent as I’ve verified it both working and broken across 3 different browsers. (IE9, Firefox, Chrome).

There is also a database setting in the wp-options table that can be checked. The option name is ‘image_default_link_type’, value should be ‘file’.
