Event Espresso Ticket Purchase Issues

We are running into issues recently (nothing has changed on the site with the exception of adding additional events) on EE4, where people purchasing tickets to events are experiencing the following:

1. Individual checks out and is charged multiple times for the tickets the purchased

Ex. John Smith purchases one ticket but ends up being charged 2 or 3 additional times (in separate transactions seconds apart) when they only wanted/purchased one.

2. Individuals buy tickets and attempt multiple transactions (because initial transaction fails), the transaction fails, email is sent to customer notifying them transaction has failed but it charges the card anyway. This then leaves the transaction as incomplete in EE.

3. I have also seen a few instances where transaction is declined the first time, and the second attempt it goes through (get avs mis-match in authorize), which then leaves EE structuring the ticket as abandoned and not complete.

Your thoughts of where to start are greatly appreciated. I have also submitted a premium ticket.
