Cannot select custom questions to add to question group

I have created a few basic custom questions to add to the personal information question group. The custom questions show up as available for me to select for that question group but the site won’t let me select them…I click on them and the checkbox does not get checked and the question doesn’t get added to the question group.

We are using EE4 and we are updated with WordPress 5.

In the console it looks as though there is some sort of java error 500.

Hi,

This might be the result of a JavaScript conflict from the active WordPress theme or another WordPress plugin.
