We have a client who needs a custom registration form that includes the following: https://d.pr/i/2WZcRN – Plus a digital signature, which could just be an initial or text box to type your full name. Is this do-able in Event Espresso?

Hi there,
Sure is! Those just look like that would be added a custom questions.
The specific setup depends on how you want them to display but the student information would need to use the First name, Last Name and Email address system questions we already have built into Event Espresso (as they are the registrant).
The other questions you can add to either one or more custom question groups and select them on the event.

I would use the Personal Information question group (rename it to Student information) for the specific student questions.
The built in Address Information group for the student address details.
A custom group for the additional details such as Mothers/Fathers details, Emergency contact etc.
The signature could be a text box, we dont have a way to upload files on the registration form, however, that is possible to do using a 3rd party add-on:
