Attendee Information not showing (Version 4.2.2.reg )?

When I finish filling out the personal and contact information in “Step 1” of the Registration Checkout and proceed to payment options none of the Personal and contact information shows up? Also I see 2 empty pop up boxes on this page?

Anyone?

Can you provide a link to the site in question, so I can do a test registration?
